Barrier Failure Analysis for Aviation Investigations using IncidentXP

IncidentXP is a software tool that provides a platform for conducting barrier based investigations of incidents and accidents. It facilitates the organisation of information gathered during an investigation into a logical timeline and then structures the analysis of the event according the identification of barrier failures. Next, the immediate and systemic causes of the failures are analysed in order to generate logical and effective recommendations to improve safety performance.

It can be used as a stand-alone tool or integrates seamlessly with BowTieXP risk analysis software to establish a feedback loop between predicted performance and real world data.

Who is it for?

Any individuals conducting or overseeing serious incident or accident investigations.

This course is designed to meet the needs of those already familiar with the concepts of incident/accident investigation but want to learn how to apply a barrier based analysis approach and to use IncidentXP software.

What does the course cover?

Use IncidentXP to:

  • Construct a timeline and sequence of events
  • Carry out a barrier based analysis of the occurrence
  • Develop recommendations and generate reports
  • Create a feedback loop to BowTieXP risk assessments to improve understanding of the risk landscape


Attendees should already have completed the Across Safety 2 day BowtTe XP course or similar, or have a good working knowledge of BowTieXP.

What format will the training take?

We use a mix of presentations, interactive exercises, case studies and guided workshops to make sure you get the most out of the day.

We only choose training venues that create a positive learning environment: natural light, good food and plenty of tea/coffee.
